FOR IN HOME PARTIES: Please be prepared to have a piece of identification with you, one which matches the information you give on the client intake form. A copy will be made. This is for the safety of our therapists.
Also, all therapists travel with a body guard for their safety.
Client intake: is required for 60 and 90 minute massages.
Therapeutic/Acute Injury Sessions: please fill out both Client Intake and Intended outcome forms.
Massage Party Intake: is required to be filled out by the client organizing the event.
*additional forms may be required when scheduling a Massage party. This will be discussed during initial consultation. All additional forms will be emailed, and are required before the event.
